Rules of Entry:
- Eligibility- Open to legal residents of Ohio, Pennsylvania and New York, who are 18 years or older as of the date of their entry form. Employees and family, friends or business affiliates are not eligible. By entering sweepstakes, participants agree to be bound by the rules set for in this promotion.
- How to Enter- there will be separate monthly sweepstakes; beginning on the 1st of each month and ending on the last day of each month. To enter simply put your name, full address, age phone number wedding date and number of tuxedos needed on a 3.5" X 5" stamped postcard to "Free monthly drawing" 4150 Mayfield Rd Cleveland, Ohio 44121. All entries received prior to the first of the month will be entered into the drawing for that month. No more entries will be eligible after November 28th, 2008. No photocopies or mechanically reproduced entry forms will be accepted. Eligible entrants may also elect to enter via WWW.ACTUX.COM by filling out and submitting the appropriate on-line forms. Entries will be limited to one entry per household, per month; more that one entry per month will result in disqualification from drawing
- Prizes and Values: One winner will be selected each month to receive up to 5 Free tuxedo rentals (rentals include Tuxedo Jacket, Pants, standard shirt, vest & tie).approximate value $500.00. All tuxedos must be returned within 2 days after use. Additional consolation prizes may be awarded to other entries which may vary by month.
- Odds, Restrictions & Random Drawing: Odds will vary based on the amount of participants. Winners will be notified by mail or phone by the 10th of each month. Grand prize winners may be asked to sign a form acknowledging and verifying validation of all the rules set forth in this contest. All such documents if required will need to be returned within 14 days of receipt. Grand prize must be used within 1 year from date or the prize will be forfeited.
- General Terms & Conditions: Prize is limited to the entrant; it cannot be assigned or transferred. No prize can be substituted, no cash substitution will be allowed and no cash will be distributed. Participant may be required to sign a responsibility form for garments rented as a result of this contest. All entries on-line and mail in will become property of American Commodore Tuxedo and may be kept for future contacts or mailings.
- Winners list -A winners list may be requested by sending a self addressed envelope to "ACT Request of Winners" 4150 Mayfield Rd , Cleveland Ohio 44121. Requests must be received prior to November 28, 2008.
